Xbox Live Subscription from Amazon.com?

When you buy an Xbox Live subscription card from Amazon.com, you can either order an online code or order a card to be shipped, when you select the card option for a 12 subscription card, you find that the word card is followed by (CD) like this "Card (CD)", does this mean that a CD will be sent for activation or will I still activate the membership with a numerical code on a card???
